Step-by-step directions for completing an application for a medical marijuana card are provided by the National Medical Marijuana Program (NMMPR) at its website. You may complete the application online or request a copy of it by mail, fax, and person. The steps in completing the application are as follows:
Application From The Patient: National Holistic Healing Center
Application from the patient and his or her legal representative must be completed to the extent the patient is a resident of the United States and the District of Columbia and if so, must include the names and addresses of all persons who will have access to the medical marijuana grow operation and the persons who will be granted authorization to buy the medical marijuana.

Appointment to the National Holistic Healing Center can be requested by contacting the National Medical Marijuana Program through phone or fax or emailing or calling the program directly.
Step Three: Payment can be made directly to the Department of Revenue at 899 North Capitol St, NE, or by submitting to the NMMPR. You must also submit two sets of fingerprints to become a licensed medical marijuana user.
The National Medical Marijuana Program receives requests for an application every three months. After you have submitted your application, it will be processed within a reasonable amount of time. The National Medical Marijuana Program staff has specific guidelines that must be followed to be processed by the Department of Revenue.
Review Your Information: National Holistic Healing Center
The National Holistic Healing Center staff will review your information and determine whether the information is accurate and complete. If the information is accurate and complete, it will be sent to the National Holistic Healing Center for processing. If there are problems with the information, the application will be returned.
Once processing has been completed, the National Holistic Healing Center staff will send the certified form to you and instructions on how to mail back the certified form. You will be given three months, or more, from the date you mailed back the form to begin harvesting the medicinal marijuana. The National Holistic Healing Centers cannot process your medical marijuana until your medical marijuana card has been issued.
Destroying The Medicinal Marijuana
Once harvesting is complete, the National Holistic Healing Centers will destroy the medicinal marijuana in a secure location. It is important to note that you can get a new copy of your medical marijuana card during this period.
The National Holistic Healing Centers will send you a report detailing all of the information obtained during the time you were receiving care. This report will serve as evidence for your application.
Renewing Your Card
During this period, you must renew your card. The renewal period is not required if your medical marijuana card is expired or if your medical marijuana card has been lost.
The National Holistic Healing Center will send you a renewal notice, which you must submit to the Department of Revenue. This renewal notice is sent out each year after you first began receiving services from the center.
You must contact the Department of Revenue to request another renewal. If you have not received your renewal notice, contact the National Holistic Healing Center and ask to have a new one sent out. The Department of Revenue will process your request and send a new card to you.
The Department of Revenue should receive your renewal notice within twenty-four hours. Once the Department of Revenue receives your renewal notice, you should mail the renewal form and a renewal notice to the National Holistic Healing Center.
